Video Permission Form
This is the form we use when filming speakers for our studiocast videos. If you are filming an event hosted by the department or featuring department faculty and/or graduate students, please be sure to have the speaker(s) sign this consent form.
Instructional Recording Consent Form
This is a form for instructors who who wish to photograph, video tape, or make audio recordings in their classes. This form specifies that the purpose of such recordings is exclusively for instructional purposes and that no recordings will be made public outside of the class.

1. Login to Faculty/Staff ConnectCarolina via http://www.unc.edu/myunc/
2. Click "Self-Service"
3. In the "Faculty Center" box at lower-left, click "Grade Roster"SUBMITTING GRADES
Note: Throughout this procedure, mind the "Approval Status" box at top-right. When you begin, the toggle should be automatically set to "Not reviewed." Leave it on this setting at first.
